The fundamental mechanics of the "fight-or-flight" response to stressful situations and its knock-on effects when it persists indefinitely are actually quite well understood. Basically, thanks mainly to the stress hormone cortisol (as well as adrenaline and norepinephrine), our bodies rev up certain processes to increase blood, oxygen and energy flow to the heart, lungs, and skeletal muscles, while other processes such as digestion, wound healing, and even cognition take a back seat.
If the perceived threat or source of anxiety lingers without being abated by behavioral (e.g., exercise) or psychological methods (e.g., relaxation techniques or counseling), a number of things can happen. One is that certain nutrients can get depleted (partly to produce the hormones, partly to assist the various systems on high alert) and not be replenished properly even if you're eating well, since stress impedes digestion and absorption.
